Steps for Enrollment into the Early Intervention Program
1. Call or stop by the clinic, Monday through Friday to make an appointment
for the following Tuesday morning when you will register and have your
initial blood work and screening for TB done.
2. On Friday of that same week you will be seen by the nurse and case
manager. The nurse will read your TB test and both the nurse and case
manager will meet with you to obtain a personal history, make an assessment
and develop a plan of care.
At this time you will receive a physician appointment and an appointment for
a gynecological examination (if applicable). You will be at the clinic for
about three hours.
3. When you come to see the physician you will be examined, have your
laboratory results explained and provided with precriptions and education.
On each physician visit the nurse and case manager will meet with you to see
if you have any additional needs and to update your plan of care. You will
be at the clinic approximately two hours for your initial visit with the
physician.
