How To Utilize Our Services

The Diabetes Management Center is designed to assist patients, their families and physicians in the management of diabetes. Patients must be referred to the Center by their primary care physician who will continue to manage the patient's medical care. Education is performed both individually and in classes in both English and Spanish.
